Breast augmentation can enhance body shape, balance breast irregularities and can also be used following breast cancer surgery for breast reconstruction. The results can successfully alter the appearance of a woman’s body, helping her to feel more confident and feminine. At this time both saline and silicone implants are used for breast augmentation patients. The FDA has stated that silicone implants are safe and do not cause cancer. Breast Lift / Mastopexy: Virginia Bethesda MD

Factors such as pregnancy, nursing, and the force of gravity take their toll on a woman’s breasts as the skin loses its elasticity. The best candidates for mastopexy are healthy women who have sagging breasts. Breasts of any size can be lifted, but the results may not last as long in heavy breasts.

Breast lift or mastopexy is a surgical procedure to raise and reshape sagging breasts-at least, for a time. No surgery can permanently delay the effects of gravity. Mastopexy can also reduce the size of the areola, the darker skin surrounding the nipple. If your breasts are small or have lost volume, breast implants inserted in conjunction with a mastopexy can increase both their fullness and size.

Mastopexy is performed as an outpatient procedure. In this operation, incisions are made around the nipple and up to the base of the breast, “lollipop incision”. Working through these incisions, excess skin from the lower section of the breast is removed. The nipple, areola, and underlying breast tissue are moved up to a higher more youthful position. Sutures close the wounds under the breast and around the nipple area.

After surgery, pain is controlled easily by medication which will subside in a day or two. The objective of mastopexy is higher, well contoured breasts. Mastopexy scars are permanent, however, since incisions are made around and below the nipples, scars should not be noticeable even in low cut clothing.

Breast Reduction Surgery:

 Breast reduction is a surgical procedure to reduce and reshape large breasts, a condition known as "macromastia."   While there are a number of different operations for breast reductions, these techniques share several characteristics.  In each procedure, incisions are made on the lower breast and around the areola.  Extra breast tissue and skin are removed and the incisions are closed to minimize scarring.  The nipple areola complex (NAC), is repositioned to a higher, more youthful position.  The type and amount of scarring varies by technique and skin quality.

Breast reduction offers many benefits for women with large, sagging breasts.  Multiple symptoms including back or neck pain, and shoulder grooving can be improved with a reduction mammaplasty. Rashes and skin breakdown can be significantly improved after the surgery.  Physical activities that were limited and/or painful can be participated in comfortably after surgery.  Overall, self-image and quality of life is greatly improved.

 

THE SURGERY

Breast reduction surgery takes 2-3 hours and is done under general anesthesia in the outpatient surgical setting. Various techniques are used to reduce the breast tissue through an inverted T incision.

Skin from both sides of the breast is brought down to reshape the breast similar to a breast lift or mastopexy procedure. Excess breast tissue is removed and the (NAC) is moved to a higher and more youthful position. Male Breast Reduction

Also known as Gynecomastia, breast reduction for men is the surgical correction of over-developed or enlarged breasts. Gynecomastia is common in men of any age and can be the result of hormonal changes, hereditary conditions, or secondary to medications. It is characterized by excess localized fat and glandular tissue development.

Gynecomastia can cause emotional discomfort and impair self-confidence. Some men may even avoid certain physical activities and intimacy.

Treatment of gynecomastia can be through liposuction or direct surgical excision. Some individuals require a combination of both procedures, which can optimize results. In some men, excess breast tissue may cause sagging. This stretched skin can be tightened through a lifting procedure.

Any surgical treatment to correct gynecomastia will require incisions. While most incision lines are concealed within natural contours, some may be visible and are a necessary result of breast reduction surgery.

 

THE SURGERY

These procedures can be performed in the office under local anesthesia or in the outpatient facility under general anesthesia. Surgical selection is based on overall health. Eyelid Surgery / Blepharoplasty:

Do You Look Tired even when you're well rested?

If you answered yes, then you may want to be one of the thousands annually who improve their appearance by firming up the area around their eyes. As we age, our eyelids stretch, muscles weaken and fat accumulates, all of which can lead to "a tired look" around the eyes. Eyelid surgery treats both the loose skin and fat below the eyes, commonly called "bags."

You need to make Dr. Zahir aware of any eye disease such as glaucoma, dry eyes, thyroid disease or previous Lasik surgery.

THE SURGERY

The surgery is usually done on an outpatient basis in our office or at the outpatient surgical facility.  During the procedure, local anesthesia is used with some mild oral sedation. To minimize scarring, the surgical incisions follow the natural lines and creases of the upper and lower eyelids.  Fat, excess skin and muscle tightening are addressed during the procedure and sutures are removed in five days. In some patients, a temporal brow lift or full forehead lift is required to create facial harmony.

As people age, gravity, sun exposure and the stresses of daily life can take its toll. Patients commonly complain about their sagging neck, jowling, facial lines and skin laxity. Friends and family may ask if you are “tired or sleep deprived”, which may not be true. Often the youthful person inside does not correspond with what one sees in the mirror.

A facelift can't stop this aging process. What it can do is set back the clock, improving the most visible signs of aging by removing excess fat, tightening the neck and jaw line. A facelift can be done alone or in conjunction with other procedures such as a forehead lift, eyelid surgery or nose surgery.



THE BEST CANDIDATE FOR A FACE LIFT

The best candidate for a facelift is a man or woman whose face and neck have begun to sag, but whose skin still has some elasticity and whose bone structure is strong and well-defined. Most patients are in their forties to sixties, but facelifts can be done successfully on people in their seventies or eighties as well. Sometimes fat injections and the use of facial implants can restore facial anatomical deficiencies. The exact procedure, which is best for you, will be determined by your aesthetic goals and needs.

 

THE SURGERY

Dr. Zahir places incisions hidden in skin folds or hairlines and on the inside of the ear. He then separates the skin from its underlying tissues and tightens the superficial muscular system (SMAS) and neck muscle (platysma). Excess skin is removed and the area is sutured snugly in place. Bandages are applied and most patients go home after surgery.


This procedure is performed in a certified outpatient surgical facility under anesthesia or twilight sedation monitored by a board certified anesthesiologist. Surgical selection is based on overall health. Pain is minimal and relieved with oral pain medications. Bruising and swelling is minimal with the use of Dr. Zahir’s technique and treatment protocol, which is started 3 days prior to surgery. Patients can return to work or social situations within 7 –10 days. There are no sutures that need to be removed after surgery. The choices for minimal incision facial rejuvenation include the MACS-Lift and the Mini-S Lift. These are both good options for patients who want to look their best for an event fairly soon and who want to minimize time away from work.

 

MACS-Lift (The Minimal Access Cranial Suspension-Lift)

The advantages of this technique include the incision being limited to the crease in front of the ear and hairline above the ear. This technique has long-lasting results because the soft tissue from the upper neck, jowls, and cheeks is elevated and firmly anchored to the deep structures of the temple. In some patients, results of the MACS-Lift can be comparable to those achieved with more aggressive procedures.

 

Mini-S Lift (The Mini Short Scar Facelift)

A ‘Mini-S lift’ can erase years from a person’s appearance in a relatively short amount of time. It is also known as: short-scar face lift, S-lift and various other similarly named procedures. The Short Scar facelift procedure is a minimally invasive lift with an incision half the length of a traditional facelift incision. This procedure allows for placement of incisions in front of the ear and up to half the length of the posterior ear.

While the Mini-S face lift works well for people who cannot commit to significant recovery time, a full facelift still produces the most dramatic results because it tightens skin in the cheeks, chin, jowls and neck. This procedure is often performed in conjunction with forehead and eyelid surgery.

The Mini-S lift belongs to a group of procedures known as partial facial procedures, which can be undertaken incrementally or in combination to achieve significant yet natural-looking results. Some of the more popular partial facial procedures include: mid face lift (cheek lift), forehead lift (brow lift), neck lift (cervicoplasty) or eyelid lift (blepharoplasty). One major advantage of partial facial procedures is their shorter at-home recovery period. For example, mini face lift patients can return to work sooner than the one to two weeks required after a full face lift. However, since everyone heals differently, we recommend scheduling any facial procedures at least four to six weeks before a special event.

The outcome of both minimal incision facelift procedures provides far more than a “minimal” result. The choice of procedures depends on what your facial assessment requires. Procedure

One of today's most popular cosmetic surgical procedures is liposuction. Used to improve the appearance of the chin, neck, jowls, cheeks, arms, thighs, buttocks, knees, hips, breasts and abdomen, liposuction can improve a multitude of figure faults. The result is a thinner, healthier body that increases your confidence and self-esteem. When determining the ideal candidate for liposuction, general body condition is more important than age. While patients of all ages can benefit from this procedure, the ideal candidate practices good eating habits and exercises regularly. Age and skin quality are important to determine best outcomes.

 

LIPOSUCTION OPTIONS

The newest method of liposuction, Lipocontouring, combines ultrasonic-assisted liposuction (UAL) and power-assisted liposuction (PAL). Ultrasonic-assisted liposuction crushes and liquefies the fat cells before they are removed. Smart Lipo a newer compendium, allows for the use of small cannulas to assist in destruction of fat and subsequent removal with added skin contraction.

THE LIPOCONTOURING PROCEDURE


Prior to liposuction, tumescent fluid is injected into the areas for fat removal. The fluid helps separate the fat making it easier to remove and safer, thus minimizing bleeding and bruising. Treatment recommendations specific to your needs will be discussed during your consultation with Dr. Zahir. Many of the lipocontouring procedures can be done in the office facility, while larger volume removal may need outpatient surgery.

ULTRASONIC-ASSISTED LIPOSUCTION


This is a newer technique. Similar to suction-assisted liposuction, ultrasonic uses a hollow cannula to remove fat cells. The cannula is connected to an ultrasound generator that causes it to vibrate more than 20,000 times per second. This vibration causes the fat cells to rupture, allowing the emulsified fat to be removed easily.

POWER-ASSISTED LIPOSUCTION


This is another excellent tool to allow for removal of fat cells using a newer method of tumescent fat removal, which does not generate heat or burn skin. General intravenous or local anesthesia may be required. Recovery time can vary from a day to two weeks, depending upon the areas treated, the extent of fat removed and the general condition of the patient. Sometimes an overnight stay is required for large volume extractions.

Abdominoplasty

This surgical treatment is reserved for candidates with loose abdominal skin and a large fat deposit in the abdomen that does not respond to exercise and diet. Post pregnancy patients with skin laxity and stretch marks are also good candidates. This is a surgical procedure performed for the patient population in whom liposuction alone will not allow for skin tightening.

 

Tummy Tuck Surgery

This procedure removes up to one third of the lower abdominal wall skin, stretch marks and fat. The abdominal muscles are tightened with two layers of permanent sutures to reinforce the muscles and create a youthful and flat tummy. Combined hip and flank liposuction create a waistline that may have been lost due to pregnancy or significant weight gain. With muscle laxity and stretched skin the only treatment options for some is a full abdominoplasty. The belly button is retained during the operative procedure, but brought out in a new area of the abdomen, once it is tightened.

 

Circumferential Tummy Tuck (Belt Abdominoplasty)

This is reserved for the patient with massive weight loss, in whom a simple abdominoplasty will only address the tummy region. These patients need to have the excess skin removed circumferentially. This leaves both a vertical and horizontal incision in the front and back, comparable to a “belt”. In some weight loss patients the belt abdominoplasty helps restore a thinner waistline.

A mini tummy tuck is different from a full tummy tuck in three typical ways. First, only the lower abdominal and fat are addressed. The upper abdomen skin is not removed. Second, the belly button is NOT repositioned. Third, the muscles are not tightened, unless necessary, during a mini tummy tuck. The best candidate for this procedure is the person who only has some loose skin and fat in the lower abdomen but not the upper abdomen. The scar is kept as low as possible so it is easily hidden under clothing. A mini tummy tuck comes with a recovery time of one to three weeks and patients should expect to decrease their activity during that time.

Rhinoplasty / Nose Surgery:

Surgery of the nose, also known as Rhinoplasty improves the appearance and proportion of your nose, enhancing facial harmony and self-confidence. Rhinoplasty can change the nose size, width, tip, and profile and address asymmetry. Large nostrils are surgically reduced to improve both aesthetic contour and shape.

 

The Surgery

Surgery of the nose is performed either using a closed procedure, where incisions are hidden inside the nose, or an open procedure, where an incision is made across the columella (the narrow strip of tissue that separates the nostrils). Through these incisions, contouring of the nose can be performed. Once the underlying structure of the nose is sculpted to the desired shape, nasal skin and tissue is redraped and incisions are closed. You may have nasal packing that is removed on the third postoperative day.

Your results may in some cases be immediately noticeable, however you will see more of a change in 6-8 weeks following surgery, with continued refinement for 6-12 months. Cartilage is a flexible and growing tissue and some results are not fully predictable.
